As our Supported Living Supervisor you will be directly supervising Direct Support Professionals and providing quality care to individuals that we serve in an environment, which promotes their rights, dignity, individuality, and needs.

Here is what you can do for Wabash Center as our Supported Living Supervisor (SLS):

• Establishing objectives for the Person Centered Individualized Support Plan (PCISP) goals
• Assisting with Behavior Support Plan (BSP) development
• Obtaining approvals for BSP’s
• Reporting for Human Rights Committee meetings
• Preparing high-risk plans in conjunction with the Wellness Coordinator (RN)
• Writing individual served specific protocols
• Completing and submitting monthly or quarterly reviews
• Ensure utility bills and leases are handled appropriately
• Review of end-of-month paperwork for accuracy
• Attending appointments and administering medications as needed
• Provide support to Direct Support Professionals (DSPs) within assigned locations as delegated and trained by the Senior Program Manager
• Complete Performance Evaluations and manage DSP attendance through approving timecards and assigning points
• Responsible for filling open shifts with DSP staff and being available to cover shifts and provide on-call support in assigned locations as needed in support of service site needs
• Assist DSPs by supporting the division of shift responsibilities and tasks to ensure proper care of the individuals that we serve
